Difficulty Logging In Using a Patient ID

If you cannot log in using your Patient ID, you may have already activated your 4myheart.com patient account.


The Patient ID is used only for your initial login. After activating your account with your Patient ID you should have been prompted to select a username and password to protect your confidential health information.
