Finding your Patient Identification Number

If you are a patient and trying to login for the first time, you need to log in using your Patient ID.

To obtain you Patient ID, please call our Client Services department at 1-800-Heart-89 (800-432-7889).

After you have your Patient ID, log in to 4myheart.com entering your Patient ID in both the Username and Password fields. You will then be prompted to verify your account by entering your last name, Patient ID number, and date of birth. After this you will be asked to update your username and password to protect your confidential health information, after which you will no longer need to use your Patient ID to log in to 4myheart.com.
